How to add forecasts (virtual bets) on site ?

1 field : date and day of the beginning of event
If Spartak - the Dynamo plays on October, 1st, 2009 and you this match have
chosen that for the forecast write so: 01.10.2009
2 field : Time of beginning of match
Here we enter an exact times of the beginning of event. For example 17:00
3 field : Event or match
Here enter the match name , only the name of playing teams. For example: Spartak
- M Dynamo At all do not write here the Championship of Russia 1 round and so
on. Only the name of a match in the form of playing teams
4 field : Correct score of match
Here we write possible correct score of the match. The given field does not
influence on what. It is interesting to other people to know your opinion .
For example 2-1 or 0-0 or 107-89 in basketball
5 field : Your bets
Here on what you wish to bet , and that should moderators check as
win or losse the rate. For example: the Victory 1, or yellow cards more than 3.5
